Instructions for Downloading, Filling Out, and Delivering Form

4. Deliver The Form. We have two delivery Options: The form must be submitted for processing by either delivering it personally or mailing it to our location.

5. Submission: Ensure the form is properly filled out and delivered either in person or by mail for processing. You will receive an email to the address on your account to confirm the receipt and status of your request.
Cancellation Form
During the Initial Term:
In order to request cancellation of the Membership Agreement during the Initial Term, a completed copy of this form must be submitted to Proactive at least 30 days prior to the requested cancellation date, along with one of the following:
(a) written proof (e.g., copy of driver's license or utility bill) that Member has changed their permanent residence to a location more than 25 miles from Proactive;
(b) a written statement from your medical physician verifying Member is unable to receive massage services due to health related reasons;
(c) written proof (e.g., death certificate) of the death of Buyer or Member.
During Month-To-Month Term:
During the month‐to‐month term of this Agreement, a Member may cancel the Membership Agreement for any reason with 30 days’ written notice. To request the cancellation of your Month-To-Month Membership Agreement, a completed copy of this form must be submitted to Proactive at least 30 days prior to the requested cancellation date.
Freeze Form
Proactive Massage + Bodywork, LLC offers our Freeze option to members that may need to temporarily suspend their Membership Plan due to extenuating circumstances, such as extended illness, military leave, or temporary relocation. During the Freeze period, the member will not be required to make any of the regular monthly payments required under the Massage Membership Plan Agreement. During the Freeze period the member will not accrue any additional plan benefits including monthly Membership Plan Massages.
A Membership may Freeze for a minimum of one (1) month and a maximum of three (3) months. Memberships can only be frozen two (2) times in a twelve (12) month period for a maximum of three (3) months at a time. A membership must be unfrozen a minimum of thirty (30) days before it can be frozen again. A freeze fee of $10 will be charged during the freeze period. A minimum of one (1) automatic payment must have been made before a Membership is eligible to be frozen. The Initial Term of the Member’s Membership Agreement will be extended by the length of the frozen period.
Freeze Plus Form
Proactive Massage + Bodywork, LLC offers our “Freeze+” option to members with at least six (6) accrued but unused monthly membership plan massages. This option allows a member to pay a very reduced fee of $10 per month for up to a six (6) month period during which they can "catch up" by using their accrued but unused Membership Plan Massages without being required to make any of the regular monthly payments required under the Massage Membership Plan Agreement. During the Freeze + period, the Member will not accrue any additional plan benefits including monthly Membership Plan Massages.
